At Revel Adult Day Health, nurses manage the daily health care of elders and other adults with medical conditions, providing caring support for any medical need. Nurses administer medications and monitor each adult’s overall health in order to help them manage chronic illnesses such as diabetes, COPD and hypertension. They work in partnership with other program staff, including case coordinators, to implement the participant’s individualized plan of care and ensure that all of their medical needs are being met on a daily basis.
