Case Coordinators

Case coordinators manage all of the services a participant receives at their center. Case coordinators may make medical appointments, oversee rehabilitative care and monitor diet to ensure it is meeting medical needs. Coordinators also work with nurses to ensure that each person’s plan of care is being followed. Case coordinators help with other needs as well. If an individual needs help with housing, applying for citizenship or choosing activities that suit their interests, they can count on their case coordinator to help them access the resources they need to thrive.
