How We Help

MENTOR Adult Day Health knows that as people age, their needs change. That can mean difficulties with daily living tasks, mobility challenges, social isolation and more. Our vibrant day programs provide the medical care and daily supports that help participants remain in their homes or with family instead of requiring higher levels of care, including nursing homes. We also offer a wide range of social activities designed to help elders make friends, enjoy hobbies and feel connected to their community.

High-Quality Nursing Care in a Dynamic Social Environment

At each of our adult day health centers, our nursing team and other staff care for individuals with a wide range of needs, including:

  • Chronic illnesses such as diabetes, COPD and high blood pressure
  • Dementia
  • Intellectual and developmental disabilities
  • Rehabilitative challenges following stroke or injury

We support adults from many cultures, including Haitian, Cape Verdean, Russian, Indian, Korean, Chinese and Spanish-speaking. With the help of caring bilingual staff, our participants enjoy healthy, delicious meals and social activities that reflect their cultural heritage, including games and holiday celebrations. At our centers, participants can feel connected to their home culture while making lifelong friends.

MENTOR Adult Day Health is part of  The MENTOR Network, a national network of local health and human services providers in 37 states. Ours is a network of shared values and resources, a nationwide team of professionals determined to help every individual thrive in a community setting through innovative, flexible programs that improve quality of life.
