Our experienced Case Coordinators manage all of a participant’s services to ensure that their plan of care is being followed and their medical, rehabilitative, social and other needs are being met. Case Coordinators work closely with the elders they serve and their families with supports including:
- Coordinating medical and rehabilitative services
- Monitoring nutrition
- Supporting participants in choosing stimulating activities
Helping participants access community resources related to health, housing, financial management, citizenship, etc.
