Who We Serve
MENTOR Adult Day Health provides vibrant day programs for elders with complex medical or rehabilitative needs. On-site nurses provide medication management and daily care for illnesses such as diabetes and COPD. Services are tailored to each person’s medical, social and other needs so they can make friends while staying active and independent.
